This service is provided for the exclusive use of donors of the Community Foundation for Muskegon County, who have a Donor Advised Fund. Agencies or Organizations wanting to inquire about grant applications should contact one of the Grant Department Staff Members.

PDF printable form: Donors to CFFMC can download the "Grant Suggestion Form" in PDF Format. Fill in the Fund Name and the grantee information with the gift amount.

Please sign the form and include your phone number and the date. Please fax the form back to CFFMC at Fax: (231) 722-4616. You may also e-mail your grant recommendation to donorgrants@cffmc.org using the following language. "I suggest the following grants from the (your fund name here).
