Grant Recommendation Form

This service is provided for the exclusive use of donors of the Community Foundation for Muskegon County, who have a Donor Advised Fund.  Agencies or Organizations wanting to inquire about grant applications should contact one of the Grant Department Staff Members.

PDF printable form: Donors to CFFMC can download this Grant Suggestion Form in PDF Format. Fill in the Fund Name and the grantee information with the gift amount.  Please sign the form and include your phone number and the date.  Then fax it to us at (231) 722-4616.
